within 72 hours after your admission to our center, you receive a call from one the members of our admission team to schedule an appointment to fill out your admission form. We will go over all the details of your California Standard Admission Agreement, describe your insurance benefits and confirm the information that relates to your admission to our facility. We’re also happy to answer any questions you may have regarding our center or the services we offer. You may also visit this link to read the answers to our commonly asked questions.

The Admissions office is just off within the Main Lobby through the double doors. We invite you to visit us to make an appointment, or bring the below documents for us to print.
